For Physicians and Referring Providers
We welcome referrals from physicians, gynecologists, urogynecologists, gastroenterologists, colorectal surgeons, and any other provider caring for patients with pelvic floor dysfunction or chronic pelvic pain. To refer a patient, please fax a physical therapy referral for pelvic floor therapy to 561-599-5109. We will reach out to your patient promptly and keep you informed of their progress with their consent.
